SCUBA Reviews/Refreshers

Meet at Seatasea's Store at 12 noon on the day of the course and we all go to the pool at the same time.   Please bring your C-Card and logbook to Seatasea with you.  (We cannot take you into the course without seeing your Certification Card)  After completion we will be placing a completion decal on your C-card and also sign-off your log book.

You will need to complete the following forms prior to class:

• Seatasea Student Record file - (If you have not trained with us before) We will give this to you on Saturday when we see you to complete it or you can stop by and pick it up ahead of time.
• Certified Diver Experience Program Release (please download in .pdf
• PADI Safe Diving Practices (Please download in .pdf) 
• Medical Statement (Please download in .pdf)  If you check a yes in any of the box's we will require a physicians release.  If not, just sign the form and bring it.

 Register on-line.  The following forms will need to be completed prior to class, so please bring them. 

You will need to complete the following forms prior to class.   The Student Record File is available at Seatasea.  We can also mail it to you...just let us know.   Upon completion we will be signing off your logbook and affixing a decal indicating you have completed a Scuba Review.

Important:  If you are certified by another dive certification agency, this course does not award you a PADI Certification Card.  If you are interested in a PADI Certification rather then then one you have, please talk to us for details.
